Action Planning Sessions
These sessions are standalone pieces of therapeutic work that aim to address and problem solve a particular behaviour, usually, but not restricted to a health-related behaviour. They are aimed at adults and are particularly effective for professionals who are used to strategic thinking and planning. Using an evidence based approach the work begins with a thorough physiological and psychological assessment of current behaviours in order to identify any issues and areas for improvement; often these are not what they appear to be and may be a consequence of other unidentified behaviours. To make the face to face session as productive as possible some of this information will be gathered prior to the session via electronic forms so that it can be analysed in advance.
The assessment is proceeded by a formulation of your needs and aspirations where we will consider all six ‘Core Elements’ of the Heart and Head Model in relation to your goals. By the end of the session we will have developed a clear idea of your goals and an Action Plan that takes into consideration the barriers you may face in implementing your plan, both physical and psychological, as well as solutions to overcome these. Where appropriate, the work may also involve an element of Psychoeducation which aims to educate the client about the theoretical underpinning of their presenting issues in order to gain a deeper understanding of their condition or behaviours.
Following the session you will receive an electronic written report and Action Plan within 48 hours, and then a 30 minute online or telephone call 8 weeks later to review the effectiveness of the plan and to address any issues you may have encountered.
Action Planning sessions are most effective when individuals recognise they are motivated and ready for change and are ready to engage in the process. Research shows that a 50/50 collaborative process between the practitioner and client leads to the best possible outcomes.
